You provide us with most of the information we need to process your policy or claim on the application for insurance, loss notice, other forms or by telephone. This information may include your name, address, telephone number, social security number, driver’s license number, and date of birth, employment, gender, marital status, and prior insurance information.
Third Party Information
This is information we receive to verify or supplement your application or loss notice. This information may come from property inspection reports, credit reports, medical reports, motor vehicle reports and your agent or broker.
Your file may contain information that we develop and maintain based on transactions and experiences you
have with us. This may include account balances, payment history, and loss history.
We may disclose certain information about you to third parties with your consent or at your direction. We may provide certain information about you to nonaffiliated third parties without your permission for the conduct of our business or as required by law. Access to and Correction of Information
Upon your written request, we will make available certain information we have collected for your review. Information collected in connection with, or in anticipation of, any claim or legal proceeding will not be made available. You have the right to request correction of any inaccurate information. We will promptly update or correct any erroneous information.
